MEDICAL INSURANCE - Eligibility: Employees
who work 25 hours or more per week, their spouses, same-sex domestic partners
and dependents of eligible employees. Health Partners provides family Networks’
group health insurance. Family Networks contributes 80% of the cost for
employees, 55% of the cost for dependents and 35% of the cost for spouses and
domestic partners.
DENTAL INSURANCE – Eligibility: Employees who
work 25 hours or more per week, their spouses, same -sex domestic partners and
dependents of eligible employees. Family Networks contributes 80% of the cost
for employees. All payroll deductions for the employee portion of the dental
insurance may be taken as pretax deductions.
PRE-TAX
REIMBURSEMENT ACCOUNTS - Eligibility: employees who work 25 hours or more per
week
Employees may
elect to make pre-tax contributions for reimbursement of out-of-pocket
medical/dental expenses and/or dependent care expenses.
LIFE INSURANCE
- Eligibility: employees who work 25 hours or more per week. Group Life
insurance is provided through MetLife, and includes
additional Accidental Death/Dismemberment (ADD). Employees are insured at 2
times their annual salary with a maximum benefit of $75,000 for each (Life and
ADD). There is no cost to the employee.
LONG TERM
DISABILITY INSURANCE - Eligibility: Employees who work 25 hours or more per
week. L-T-D insurance is also provided through MetLife. There is no cost to the employee.
PRETAX
RETIREMENT PLAN - Family Networks 403(b) Thrift plan allows for pre-tax employee
contribution for retirement savings. Employees who meet the eligibility
requirements will receive an employer matching contribution
