How To Make A
Referral
DAY
TREATMENT
Programs in Minneapolis and
New Hope
 |
For information about day
treatment programs, call Day Treatment Programs supervisor at 763-504-8323. |
 |
For county authorization,
call the Hennepin County Front Door at 612-348-4111 and request Family Networks
Day Treatment Programs. The parent/guardian will also need to call. |
 |
Forward to the authorizer
clinical information regarding the client including: psychological
assessment documenting a Severe Emotional Disturbance, Individualized Education
Plan and a social history. |
 |
The county authorizer reviews
the clinical information and makes a determination regarding authorization. |
 |
The authorization and
clinical information is forwarded to the Family Networks Supervisor. An Initial
Interview is then scheduled with the client and parent/guardian. The final
determination of day treatment placement is made following this meeting. |
OUTPATIENT SERVICES
Providing in-home therapy throughout Hennepin County and in-school therapy to
families in Northwest Hennepin County
 | Call Family Networks
Outpatient Services supervisor at 763-515-2451 with referral information. (MAY
require Release of Information form authorizing disclosure to Family Networks).

 | Send/fax any supporting
documentation. (REQUIRES Release of Information form authorizing disclosure to
Family Networks).
|
 | Have parent(s) contact
the Family Networks Outpatient Services supervisor.
|
 | If appropriate, a Family
Networks therapist will be assigned and services will be scheduled.
|
EARLY INTERVENTION AND PREVENTION
Therapeutic preschool and parenting program.

The
parent(s) must be experiencing significant stress related to parenting and/or
stress that significantly impacts their ability to parent. |
 |
The
family can be referred by an agency, clinic, school district or self-referred |
 |
The
family must have a child (children) between the ages of 0-6. |
 |
The
family must reside in the NW suburban Hennepin County area or North Minneapolis. |
 |
The
parent participates in an intake sessions, in center, to evaluate need and all
services are authorized by Hennepin County. |
 |
The
parent(s) must be willing to participate in center-based parenting classes at a
rate of at least one session per week. |
 |
The
parent(s) must be willing to participate in weekly in-home visits. |
